SOLVED:  Appears someone windows authentication turned off in IIS.   Turned on and working correctly. 

On premise with Active Directory sync created previously working for clients with windows authentication.  Appears to now be asking for username and password in Chrome starting today.   Unable to login.   Any advice?  

I have talked to support already regarding this issue and was informed issue due to fillable forms being used for merge forms.  Replaced forms with non fillable versions and problem resolved. 

Thanks for the responses.  I believe Phil Robson's response should do it.  For some reason overlooked that setting and was set incorrectly. 

Have created an email notification to go out that sends a Date/Time Index Field as part of the description for the email.   The time appears to be received as the incorrect time on these emails and are 7 hours off in time.  Possible a Time Zone issue?  Please advise on how to correct.   Verified Index field shows correct date/time but email is showing incorrect time.  

Trying to use a form to populate multiple merge forms.  Most work as designed but a couple appear to be placing the data in random spots.  Tried recreating the failing merge forms but same issue occurs.  Attaching copies of screenshots as example.   Any ideas to correct appreciated.
